Dr. Madrilejo
Endocrinology & Internal Medicine
Dr. Madrilejo has been serving patients in Bakersfield and Kern County since 1998 — more than 20 years of dedicated care. A pioneer in telemedicine since 2005, his expertise spans hormone-related disorders, diabetes, insulin imbalances, thyroid diseases, and other endocrine conditions. As an internist, he also cares for adults and adolescents with chronic and complex conditions including hypertension, obesity, heart disease, and chronic lung disease. Dr. Madrilejo is committed to comprehensive care that emphasizes accurate diagnosis, effective treatment, disease prevention, and long-term health management.
Carlos Alvarez, MS, PA-C
Physician Assistant
Mr. Alvarez is an outstanding, popular, and bilingual Physician Assistant board certified by the National Commission on Certification of Physician Assistants. He earned his Bachelor of Science in Biology with honors from CSUB in 2003, followed by his Master of Science in Physician Assistant Studies from Western University of Health Sciences in 2006. For the past ten years, Mr. Alvarez has been delivering high-quality medical care to patients throughout Kern County. He and the other long-time providers at Centennial Medical Group are all accepting new patients.
Michelle Magugat, FNP-C, MS
Family Nurse Practitioner
Michelle Magugat is a Board Certified Family Nurse Practitioner with over 26 years of experience providing direct patient care to individuals across a broad array of medical settings in Kern County. She completed a Bachelor of Science in Nursing and a Master of Science in Nursing, and is a highly qualified medical professional and a valued addition to Centennial Medical Group. Michelle is accepting new patients.

Lilian Mechael, PA-C
Physician Assistant
Lilian Mechael, PA-C, is a dedicated Family Medicine Physician Assistant committed to providing compassionate, patient-centered care. After graduating from Medical School in Egypt, she relocated to the United States and continued her medical journey through the Physician Assistant Program at the University of California, Davis. With a unique background as an internationally trained Physician and a U.S.-trained Physician Assistant, Lilian brings a strong foundation in patient care, clinical decision-making, and evidence-based medicine. She believes in treating every patient with kindness, respect, and understanding while building meaningful relationships with the individuals and families she serves. As a wife and mother of two children, Lilian understands the importance of family and the role good health plays in everyday life. She enjoys spending quality time with her family and creating lasting memories. She feels honored to serve her community by helping patients achieve healthier, happier lives.
Dr. Tom Nelson, DPM
Podiatry
Thomas Nelson, DPM, is a board-certified podiatrist who specializes in diabetic foot care and podiatric surgery. He received his doctorate degree from the California College of Podiatric Medicine and completed his preceptorship with Robert Garner, DPM. Dr. Nelson has been in private practice for more than 30 years, serving the Bakersfield area.
